​What to Bring on Your First Visit

We ask that you bring your completed new patient paperwork (listed below) and your dental insurance information to your first visit. If you prefer to complete the new Patient Paperwork in the office, please arrive 15 minutes early for your appointment.

You might also may want to write down any questions or concerns which you have about your oral health, and bring this list in with you. This will help to make sure that all your concerns are addressed.

During Your First Visit

Your first visit will include the following:

• A review of your complete dental and medical history form

• A review of your dental x-rays and/or taking new x-rays as needed

• A dental exam

After a thorough exam, Dr. Aftab will discuss his findings and, if necessary, all options for treatment. His goal in this discussion is to help you come to an informed decision regarding your dental health.
